Office Policies

 

1. Children can be examined at age 6 and above.

2. Children under 18 must be accompanied by an adult during the entire eye exam.

3. For a Motor Vehicle form to be completed you must have a full eye exam. Patients can go to the DMV for a free screening or get a complete exam at this office. We have the forms here.

4. First-time contact lens wearers must make a one hour appointment and must be above the age of 13.

**First-time wearers can only be scheduled Monday-Saturday and TWO HOURS before   closing.
